This Notice of Privacy Practices (“Notice”) describes how Aletheia Health LLC d/b/a Aletheia Telehealth (“Aletheia Telehealth,” “we,” “our,” or “us”) may use and disclose your health information, and your rights regarding that information.

4. Other Uses and Disclosures Requiring Authorization

Certain uses of your PHI will only be made with your written authorization, including:

Marketing communications not permitted by law.

Sale of your PHI.

Psychotherapy notes (with limited exceptions).

You may revoke your authorization at any time in writing, except to the extent that action has already been taken based on it.

5. Your Rights Regarding Your PHI

You have the right to:

Access – Request a copy of your medical records, in paper or electronic format.

Amend – Request corrections to your health information if you believe it is inaccurate or incomplete.

Accounting of Disclosures – Receive a list of certain disclosures of your PHI made in the past six years.

Restrictions – Request restrictions on how we use or disclose your PHI (though we are not required to agree in all cases).

Confidential Communications – Request that we contact you at a specific address or phone number.

Paper Copy – Request a paper copy of this Notice, even if you agreed to receive it electronically.

File a Complaint – File a complaint if you believe your privacy rights have been violated (see Section 7).

6. Changes to This Notice

We reserve the right to change this Notice at any time. Changes will apply to all PHI we maintain. Updated versions will be posted on our website and available upon request.
